AmeriCorps Member is responsible for completing the form below to
document all hours served. |

When
documenting hours, include the dates and times (include AM and PM) of service listed Monday
through Sunday. Hours should be separated into training hours and regular service hours. Training hours are
defined as any hours in which you are trained to perform your service. Any preparation time should
also be recorded under the training hours and
described in the last column as preparation. Fundraising hours are hours spent involved in a community or
school-based fundraising events for example a Book Drive or Hurricane
Katrina collection efforts. |

Upon
completion of this form, please hit the calculate button at the bottom
of the page. A confirmation page will appear which the member
is required to print and submit to his/her supervisor for signatures.
All timesheets must be signed in BLUE INK
and must be received in the SMILES AmeriCorps office
NO LATER than the 5th of the following month.
Once timesheets are submitted and approved members cannot submit
additional hours or change hours on their timesheet. It is also recommended that
each member retain a copy for his/her records. |
